3. If it is a donation - please fill out the application and mail it with a check or money order made payable to Dean Vereen/Medical Assisted Treatment. At the bottom of the check or money order where it says For_______write in Donation. Remember we are a 501(c)(3) non-profit organization and your donations are tax-deductible.

4. If you do not have a printer then feel free to send a money order or personal check to the address given below. All donations go toward membership and once you have donated thirty-five dollars (one time donation) ; you will always be a member. It is tax-deductible and make sure you write it out on your check or money-order and you will receive a receipt from us once a year with your total yearly donation for tax purposes.
